Your First Visit
At your first appointment, we’ll help you complete the necessary paperwork (if you haven’t already). Dr. Parks will be ready to see shortly after we’ve confirmed that everything is in order. While examining your teeth, we’ll ask questions about your current situation so that we have an idea of what your current oral health concerns and goals are. Follow-up appointments can be scheduled as necessary based on what we find while examining and cleaning your teeth.
New Patient Forms
When visiting any dental office for the first time, you’ll need to fill out a number of forms detailing your current oral and overall health. This information lets us personalize your treatment plans according to your needs.
